﹡
Insurance Paid Session Fees
Clients will be responsible for the copay and/or co-insurance of their insurance plan. If you have deductibles that have not been met, you will be paying the negotiated rate of your insurance policy, which varies with different insurance plans.
﹡
Cancellation Policy:
If you are unable to keep your scheduled appointment, please call / text to cancel as soon as you can or at least 24-hour ahead of time. A full fee will be charged for no show without 24-hour advanced cancellation.
﹡
Herb / Supplement Return Policy:
Unused or partially used herbs / supplements cannot be returned because each formula is customized for an individual patient and medical condition. Furthermore, once herbs are mixed it is impossible to separate the ingredients.
﹡
Insurance Acceptance:
We accept many insurance plans that cover acupuncture. You can provide your insurance information to us when you call to make your first appointment. If you would like to call your insurance company to find out the details of your coverage, you may ask your insurance company the following questions:
- Do I have acupuncture benefits?
- How many visits are covered in a year?
- How much is my copay and/or co-insurance?
- Do I have deductibles I need to meet?
- Do I need a referral or pre-authorization for my visits?
- Are there any restrictions in terms of conditions covered by acupuncture policy?
Our insurance acceptance policy is subject to change.
﹡
Payment Methods:
Payments are due at the time the services are rendered. We currently accept personal checks, cash, visa, and master cards.
﹡
Returned check policy:
A charge of $50 and any additional amount incurred due to a bounced check will apply should your check be returned due to insufficient funds.
